Guest Ticket Orders

All COA members in good standing will automatically receive free High Holy Day tickets based on your
membership category (1 Adult or 2 Adult memberships include all dependent children up to age 29).

Guest tickets are required for any children aged 30 and older, as well as for any family members or guests who will be joining you for the holidays.

The cost of Guest Tickets will be $150 per person for either Rosh HaShanah or Yom Kippur or $250 for both
High Holy Days.

Please complete and return this form by Wednesday, September 17 at 12:00pm. All tickets will be emailed!
Requests received after this date can be picked up at the Welcome Table, in the lobby, on the day of services.
Please make checks payable to COA and return to 708 Ridge Pike, Lafayette Hill, PA 19444. If you want to
pay by credit card, contact the office at (610) 828-9066.
